Caregiver Support Program - Registered Nurse

The incumbent will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent will conduct initial and annual home visits to Family Caregivers approved in the program and will conduct the Veteran Functional Assessment Instrument (VFAI) and other assessments as needed via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. The incumbent possesses advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. The incumbent demonstrates proficiency in practice based on conscious, deliberate planning. Self-directed in goal setting for managing complex patient situations. The Clinical Assessor RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. The Clinical Assessor RN is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Clinical Assessor RN must be able to push-pull and lift 35 pounds and must be able to stand for prolonged periods of time. Finally, the Clinical Assessor RN is expected to work in a collaborative, team-oriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Fostering positive interdisciplinary collaborative relationships is essential for continuity of care that is safe and quality driven. The Clinical Assessor RN functions within a nursing process framework for problem-solving and care facilitation:

NURSE II EDUCATION EXPERIENCE DIMENSION REQUIREMENTS BSN - 2-3 years experience (OR) Associate - 2-3 years experience Degree or Diploma in Nursing and bachelors degree in a related field (OR) Master's degree - 1-2 years experience in nursing or related field with BSN or bachelor's degree in a related field. If MSN obtained in a Bridge Program, no BSN required. (OR) Doctoral degree - 0 years experience in nursing or meets basic requirements for appointment and has doctoral degree in related field. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
